The BEA-BEST trial is a study that aims to improve how we diagnose acute respiratory failure, which is a serious condition where patients have trouble breathing and may need extra oxygen. Researchers at the University of Louisville are looking to see if using advanced technology called Eko AI, alongside a standard lung examination method, can help doctors make better diagnoses for patients in this situation. The study is currently recruiting participants, and anyone aged 18 or older who comes to the Emergency Department with new breathing difficulties may be eligible to join.

If you or a loved one is considering participation, you will need to provide consent, and your condition should have started within the last week. Participants will have their lung sounds recorded with a stethoscope, which helps doctors understand what might be happening in the lungs. However, some people may not be eligible if their breathing issues are caused by trauma or certain other conditions. This study could help improve future treatments for those experiencing acute respiratory failure, making it an important opportunity for both patients and the medical community.

Eligibility criteria

  • Inclusion Criteria:
  • Adults aged 18 years or older presenting to the ED or admitted to the hospital within 12 hours of enrollment
  • Acute respiratory failure defined as new onset shortness of breath initiated within the last 7 days, and new or increasing need for oxygen therapy
  • The patient or patient's legal health care proxy consents to participation
  • Exclusion Criteria:
  • Unwillingness to consent
  • Patients with trauma as the cause of ARF
  • Patients with pneumothorax as the cause of ARF
  • Inability to perform pulmonary POCUS or lung auscultation (e.g. dressing on the chest)
  • Unwilling or unable to complete the minimum of 12 lung sound stethoscope recordings.
