This clinical trial is studying patients who come to the emergency department with a serious condition called acute hypoxemic respiratory failure, which means they are having trouble getting enough oxygen into their lungs. The researchers want to learn more about the different types of this condition and what factors—like clinical signs, imaging results, and biological markers—can help predict how likely a patient is to recover or experience ongoing severe breathing problems. They hope to gather important information that could improve care for future patients.
To join this study, participants must be adults over 18 years old who are experiencing new episodes of acute hypoxemic respiratory failure and need a significant amount of oxygen to maintain their oxygen levels. Unfortunately, people under 18, those in the hospital for comfort care only, or those with certain other types of respiratory failure will not be eligible. If someone participates, they can expect to have various health information collected early on, which may help doctors understand their condition better. The trial is currently recruiting participants, and all genders are welcome to take part.

Eligibility criteria
- Inclusion Criteria:
- • Adult patients (aged \>18 years) presenting in the emergency department
- • De novo acute hypoxemic respiratory failure (requiring oxygen flow rate of 5 liters per minute or more to maintain SpO2 of 90% or more)
- Exclusion Criteria:
- • Age \<18 years
- • Not admitted to the hospital
- • Postoperative acute respiratory failure (within one week from surgery)
- • Chronic hypoxemic respiratory failure (requiring long term oxygen therapy at home)
- • Hypercapnic respiratory failure
- • Transferred from another hospital or facility
- • Pregnant women
- • Admitted to the hospital purely to facilitate comfort care
- • Lack of informed consent
