This clinical trial is studying two different surgical approaches for men with localized prostate cancer. Researchers want to compare the traditional surgery called radical prostatectomy with a newer method called pelvic fascia-sparing radical prostatectomy. They aim to see if both methods are equally effective in controlling cancer while also looking at how they affect patients' quality of life, particularly concerning urinary function and sexual health. The hope is that the new method will lead to fewer problems like urinary issues and penile changes after surgery.
To participate in this trial, eligible individuals must be male, aged between 40 and 80, and scheduled for radical prostatectomy. They should be able to understand and communicate in English or Spanish and be willing to follow the study rules. People who have had major pelvic surgery before or have certain complications won't be able to take part. Those who join can expect to be randomly assigned to one of the two surgery types and will be monitored for their health and quality of life after the procedure. This research could provide valuable insights into better surgical options for prostate cancer patients.

- Inclusion Criteria:
- • Male sex
- • Age ≥40 years or ≤80 years
- • Scheduled for radical prostatectomy for clinically localized prostate cancer
- • Able to read and speak English or Spanish
- • Willingness to sign informed consent and adhere to the study protocol
- Exclusion Criteria:
- • Prior major pelvic surgery or radiotherapy
- • Suspicion of N1 disease (i.e., any lymph node greater than 1cm in maximal diameter)
