**Summary of the Clinical Trial: Remote Monitoring to Prevent Frailty Progression in the Elderly**
This clinical trial is looking at ways to help older adults who are "pre-frail," meaning they are starting to show signs of frailty but are not yet fully frail. Frailty can make it harder for individuals to perform daily activities and can lead to serious health problems. Researchers want to see if using a wearable device to monitor health can help these individuals stay healthier for longer. The goal is to develop a program that can be used in communities, especially in small towns like Salcito in the Molise region, to support healthy aging.
To participate in this trial, you must be at least 65 years old, live in Salcito, and have been diagnosed as pre-frail. Unfortunately, if you have certain physical or mental health conditions, or if you have a pacemaker or other medical device that makes using a wearable device difficult, you won't be able to join. If you are eligible and decide to participate, you can expect to be monitored remotely for your health status, which could help improve your quality of life and prevent further decline in your health. This study is currently recruiting participants, and your involvement could contribute to important findings that may help others in the future.

Eligibility criteria
- Inclusion Criteria:
- • Diagnosis of Pre-Frail;
- • Salcito (a small mountain village in Molise region) residents.
- Exclusion Criteria:
- • Age\< 65 years;
- • Individuals affected by physical or mental diseases, pacemaker or other medical device keeper who make the use of "wearable device" not possible or unable to complete functional tests.
