This clinical trial is studying a new program designed to help patients recover better after spending time in the intensive care unit (ICU). Many people who leave the ICU face ongoing challenges like muscle weakness, poor nutrition, and mental health issues, which can affect their overall quality of life. The trial aims to see if a 12-week lifestyle intervention that combines group physical therapy and nutritional support can improve physical and mental health in long-term ICU survivors.
To participate, individuals must be at least 18 years old, have spent more than 48 hours in the ICU, and be between 6 weeks to 6 months post-discharge. They should also have a specific score indicating physical challenges. Participants in the intervention group will attend physical therapy sessions twice a week and receive guidance on improving their diet. They will also complete health questionnaires and undergo some physical tests during the study. While there's a commitment of time and effort, the potential benefits of feeling better and improving daily functioning may be worth it.

Eligibility criteria
- • Inclusion criteria Long stay post-ICU patients (length of stay ≥48h) between 6 weeks and 6 months after hospital discharge AND \>18 years old AND able to visit the hospital 2 times a week AND a RAND-36 physical functioning subscale score \< 67%
- • Exclusion criteria
- • Allergy to components of ProSource® (Allergy information: gluten-free, lactose-free, NOT cow's milk free (wey-protein), produced in a factory in which soy, wheat and nuts are processed.
- • Inability to understand the Dutch language
- • Actively participating in a professional physical rehabilitation program during the study period.
- • Note: previous participation in a concluded rehabilitation program is NOT an exclusion criterion.
