The STAR-MAP trial is studying a new program designed to help people with high blood pressure stick to their medication and improve their overall health. This program combines personal health coaching with reminders about taking medicines, aiming to increase medication adherence, control blood pressure, and enhance the quality of life for participants. The study will involve 402 adults aged 40 and older who have high blood pressure that is not well managed and who struggle to consistently take their medications.
To join the study, you need to be a member of Blue Cross Blue Shield of Louisiana, have been enrolled for at least a year, and be able to communicate in English. You should also have been prescribed medication for high blood pressure that you find difficult to take regularly. If you decide to participate, you'll be randomly assigned to either receive health coaching along with medication reminders or just the reminders alone. Throughout the year, you’ll share information through questionnaires and health measurements, helping researchers understand how effective the program is in improving medication adherence and health outcomes.

Eligibility criteria
- Inclusion Criteria:
- • fully insured by Blue Cross Blue Shield of Louisiana (BCBSLA)
- • continuously enrolled in BCBSLA for one year
- • planning to remain a member of BCBSLA for next year
- • English-speaking
- • telephone access
- • aged ≥40 years
- • diagnosis of essential hypertension (ICD-10-CM code I10)
- • currently filling antihypertensive medication
- • low antihypertensive medication refill (proportion of days covered (PDC) \<0.8)
- • low self-report adherence (4-item Krousel-Wood Medication Adherence Scale (K-Wood-MAS-4) score ≥1)
- • uncontrolled blood pressure (BP) (systolic BP ≥130 mm Hg or diastolic BP ≥80 mm Hg)
- • desire to improve BP
- Exclusion Criteria:
- • living in a household with someone already enrolled in the study
- • enrollment in another clinical trial for drug adherence or BP control
- • moderate to severe cognitive impairment
