This clinical trial is looking at how to better support patients who are at high risk during the COVID-19 pandemic. Researchers want to see if having a System Navigator—someone who helps patients with their health and social needs—can reduce feelings of emotional distress. The System Navigator works with patients one-on-one to address not just their medical issues but also their challenges related to things like income, housing, and food security. The goal is to find out if this proactive approach is more helpful than the usual care provided in primary health settings.
To participate in the study, you need to be 60 years or older and be considered at risk due to having a chronic health condition (like diabetes or heart disease), a serious mental illness (like schizophrenia or bipolar disorder), or facing challenges like poverty or homelessness. If you decide to join, you can expect to receive personalized support from the System Navigator, which may help improve your overall well-being during these challenging times. This study will involve about 180 patients from clinics linked to the University of Toronto and aims to gather insights that could benefit patients now and in the future.

Eligibility criteria
- Inclusion Criteria:
- * At-risk and vulnerable patients of primary care organizations who are 60+ AND have one or more of:
- • 1. chronic condition (diabetes, CHF, CAD, COPD or asthma)
- • 2. serious mental illness (schizophrenia or bipolar disorder)
- • 3. anticipated to live in poverty and/or be homeless/underhoused
- Exclusion Criteria:
- • patients unable to provide informed consent
