This clinical trial is looking at a new surgical treatment to help people who have trouble walking (or limping) after having hip replacement surgery. Sometimes, even after the surgery, people continue to experience limping due to weakness or injury in a muscle called the gluteus medius. This study aims to see if surgery, followed by physical therapy, can improve hip function and reduce limping more effectively than just physical therapy alone.
To participate in this trial, you may be eligible if you have had persistent limping for at least a year after your hip replacement and an injury to the gluteus medius muscle that has been confirmed by an ultrasound or MRI. You should also have only a small difference in leg length or hip measurements. Participants will receive either the surgical treatment or continue with non-surgical therapy, and all will engage in physical therapy to aid recovery. This study is currently recruiting participants, and it's open to anyone aged 18 and older, regardless of gender.

Eligibility criteria
- Inclusion Criteria:
- • Persisting limping with positive Trendelenburg's sign for at least 12 months after total hip arthroplasty
- • Rupture/avulsion of the gluteus medius tendon verified with ultrasound or MRI
- • Leg length discrepancy of less than 1 cm
- • Femoral offset discrepancy of less than 25%
- Exclusion Criteria:
- • Neuromuscular disorders
