This clinical trial, called "Community Outreach for Palliative Engagement -- Parkinson Disease," is looking to understand how effective palliative care training for community doctors and telemedicine support can be for patients with Parkinson's disease and related conditions, as well as their caregivers. Palliative care focuses on improving the quality of life for patients by relieving symptoms like pain, depression, and other challenges they may face. Telemedicine allows patients to connect with healthcare providers using technology, making it easier to receive support without needing to visit a doctor's office.
To be eligible for this study, participants need to be over 40 years old and have a diagnosis of Parkinson's disease or similar conditions, as confirmed by their neurologist. The trial is currently recruiting participants of all genders. Those who are already receiving palliative care or have other serious health issues that require palliative services will not be included. Participants can expect to receive specialized support aimed at enhancing their overall well-being and that of their caregivers throughout the study.

- Inclusion Criteria:
- PATIENT INCLUSION CRITERIA:
- • • Over age 40 years and diagnosed with PD or other causes of parkinsonism, such as progressive supranuclear palsy, multiple system atrophy and Lewy Body Dementia by their community neurologist.
- Exclusion Criteria:
- PATIENT EXCLUSION CRITERIA:
- • Potential patient subjects who are unable or unwilling to commit to study procedures
- • Presence of additional medical illnesses which requires palliative services (e.g. metastatic cancer)
- • Already receiving palliative care or hospice services.
