Effectiveness of Multimodal Physical Therapy in Migraine

This clinical trial is studying the effectiveness of a special type of physical therapy combined with regular care for people who suffer from migraines and also have neck pain. Researchers believe that treating the neck area might help reduce migraine symptoms because neck pain often occurs alongside migraines. The trial will involve 56 participants aged 18 to 65 who experience either episodic migraines (6-14 days per month) or chronic migraines, and have neck-related issues.
Participants in the trial will be randomly assigned to either receive 16 sessions of this physical therapy over eight weeks along with their usual medical care, or just their usual care alone. To join the study, individuals should not have had any recent neck injuries or specific medical conditions that could complicate the results. This trial aims to see if the physical therapy can make a real difference in managing migraine pain and improving quality of life for those affected.

Eligibility criteria
- • Inclusion criteria
- • patients aged 18-65 years;
- • patients with episodic migraine with and without aura (experiencing 6-14 days/month with migraine);
- • patients with chronic migraine;
- • patients who achieved \>30% \<50% reduction from their individual baseline in the number of monthly migraine days on therapy with standard stable pharmacological treatments during the past 3 months;
- • concomitant cervical dysfunctions.
- • Exclusion criteria
- • self-report of neck injury/trauma;
- • acute radiculopathy;
- • history of carotid or vertebral artery dissection;
- • stroke;
- • cervical/cranial nerve block in the past three months;
- • pregnancy;
- • history of infection;
- • migraine attack during the assessment;
- • diagnosis of other concomitant headaches (such as cervicogenic headache, tension-type headache, cluster headache, medication overuse headache);
- • having received physical treatment in the past three months or botulinum toxin in the past six months;
- • any unstable medical o psychiatric condition.
- • history of any other rheumatic or chronic diseases such as fibromyalgia;
- • neuralgias, or self-report of vestibular disease.
