This clinical trial is studying how interval exercise can help improve the daily functioning of veterans who have both chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA), conditions that together are called "Overlap Syndrome." People with this syndrome can experience a decline in their physical abilities much earlier than others. The researchers want to see if a tailored exercise program can enhance participation in daily activities, increase physical activity levels, and improve heart health for these veterans.
To be eligible for this trial, participants must have a confirmed diagnosis of both COPD and OSA. They should also score above 20 on a simple cognitive test, which checks thinking and memory skills. However, individuals with certain heart conditions, joint pain that limits movement, those who have fallen more than twice in the last month, or who have been recently hospitalized cannot participate. If you join the study, you can expect to take part in an exercise program designed specifically for your needs, and your progress will be closely monitored to assess improvements in your health and daily functioning.

Eligibility criteria
- Inclusion Criteria:
- • Diagnosis of COPD and OSA, i.e., Overlap Syndrome Diagnosis of Overlap Syndrome (both COPD and OSA), confirmed in CPRS from spirometry or pulmonary function test of FEV1/FVC \<70 for COPD and a polysomnogram or home-based study for OSA.
- • Montreal Cognitive Assessment (MoCA) \>20
- Exclusion Criteria:
- • CAD, as defined by anyone of the following: history of CABG, prior positive stress test for ischemia, infarction or angina or medical problem
- • Orthopedic problems as defined by joint pain limiting ambulation
- • Fall risk, defined as more than 2 falls in the prior month
- • Hospitalization in prior month
- • Daytime home oxygen 6. Participation in structured exercise, defined by the Physical Activity Scale for the Elderly (PASE) as more than 1 hour of activity spent in moderate activity and any strenuous activity performed over prior 7 days.
