This clinical trial is studying why some people with type 2 diabetes don’t take their medications as prescribed, which can lead to serious health problems. The researchers want to compare a new urine test they developed with other methods used to check if patients are following their treatment plans. They will recruit about 600 patients who have poorly controlled diabetes from primary care settings over 15 months. Participants will provide a urine sample, fill out a questionnaire about their medication habits, and have their prescription refill rates reviewed.
To be eligible for the study, participants must be at least 18 years old, have a confirmed diagnosis of type 2 diabetes, and have a recent blood sugar level that shows their diabetes is not well controlled. They should also be taking at least one diabetes pill and one other medication for heart health. This study aims to improve our understanding of medication adherence in diabetes, which could lead to better treatment strategies in the future.

Eligibility criteria
- Inclusion Criteria:
- • Aged 18 years and over
- • Confirmed diagnosis of type 2 diabetes
- • People with at least one HbA1c ≥ 7.5% (59mmol/mol) measured in the last 6 months
- • Patients on at least one OHA and one other oral cardiovascular medication for blood pressure, diabetes or lipid lowering therapy
- • Patients able to understand written and verbal English
- Exclusion Criteria:
- • Type 1 diabetes
- • Age less than 18 years
- • Patients unable to give informed consent
- • Pregnant or planning pregnancy
- • Terminally ill
- • Patients on simvastatin and or aspirin if they are the only other medication for cardiovascular disease besides the OHAs that the patient is prescribed.
- • Patients on insulin or GLP-1 analogues if they are the only other medication for cardiovascular disease that the patient is prescribed.
- • Major medical or psychiatric illness
- • Patients unable to understand written and verbal English
