This clinical trial is studying new ways to help teenagers who have recently been in the hospital for mental health issues, especially those related to thoughts of suicide or self-harm. The goal is to combine different technology-based strategies to find the most effective support for these young people after they leave the hospital. The trial is currently looking for participants aged 13 to 17 who have either attempted suicide in the past month or have had serious thoughts of suicide in the past week.
To join the study, participants must have a legal guardian and need to own a cell phone, as the program may involve using technology for support. However, teenagers with severe mental health challenges, such as acute psychosis or those who have been transferred to a different type of care, will not be eligible. Those who join can expect to receive personalized support designed to reduce their risk of suicide and to help them transition back to everyday life after hospitalization.

Eligibility criteria
- Inclusion Criteria:
- • A suicide attempt in the last month or (2) past-week suicidal ideation (based on the Columbia-Suicide Severity Rating Scale (C-SSRS). In the absence of C-SSRS ratings in the chart, attending physician indication in the chart of the presence of suicidal ideation within the last week or the presence of a suicide attempt in the last month will be used.
- Exclusion Criteria:
- • Adolescent presenting with severe cognitive impairment or with altered mental status (acute psychosis or acute manic state)
- • Transfer to medical unit or residential placement
- • No availability of a legal guardian
- • Adolescent not owning a cell phone.
