This clinical trial is looking at two different ways to provide support to adults with cancer, especially those nearing the end of life. To join this study, you need to have recently been diagnosed with a solid tumor cancer or a blood cancer, and you should be able to understand and agree to participate. You'll also need to speak either English or Spanish. If you decide to participate, you'll have the chance to receive one of the two types of supportive care and see how it impacts your experience with cancer treatment. The trial is currently recruiting participants, and it's important to know that this study is for all adults aged 70 and older.

Eligibility criteria
- Inclusion Criteria:
- • 1. Newly diagnosed patients with solid tumor cancer diagnosis or recurrence of the disease.
- • 2. Patients must have the ability to understand and willingness to provide verbal consent.
- • 3. Participants must speak English, Spanish, Chinese, or Vietnamese.
- Exclusion Criteria:
- • 1. Inability to consent to the study
- • 2. Plans to change oncologist within 12 months
- • 3. Employed by the practice site
- • 4. Patients who anticipate moving from the area within 12 months
