The PAINT18 study is researching how adding a nutrient called arginine to the feeding of very premature infants affects their immune system. This trial focuses on babies who were born before 29 weeks of pregnancy or who weigh less than 1200 grams at birth. The study will involve 24 infants at the Neonatal Unit of Liverpool Women's Hospital, where 16 of them will receive extra arginine in their intravenous nutrition. Researchers will monitor these babies over their first 30 days of life, checking their blood for important information about how their bodies are responding to the nutrition.
To be eligible for this study, infants must be admitted to the hospital shortly after birth, but those who are not expected to survive the first week or have certain serious health issues will not be included. Parents will be asked to give their consent for their child to participate. The goal of the study is to see if arginine changes how the body fights infections and supports overall health in these vulnerable infants, which could lead to better nutritional strategies in the future.

Eligibility criteria
- Inclusion Criteria:
- • Infants born \<29 weeks' gestation
- • and/or with birthweight \<1200g
- • Admitted to the Neonatal Unit at Liverpool Women's Hospital within 48 hours of birth.
- Exclusion Criteria:
- • Infants who are unlikely to survive the first week after birth.
- • Infants known (or suspected to have) a diagnosis of inborn error of metabolism or serious liver dysfunction
- • Parents who are unable to give informed consent
