This clinical trial is studying how family caregivers can help reduce confusion, known as delirium, in older patients who have a broken thigh bone (femur) after a fall. The Hospital Elder Life Program (HELP) has previously been successful in lowering the chances of delirium in hospitalized patients, and in this study, trained family caregivers will use these helpful techniques while supporting their loved ones during their hospital stay. The researchers want to see if this support can lead to fewer cases of delirium in older adults receiving care in an orthopedic unit.
To participate in this trial, individuals need to be 65 years or older, hospitalized for a fractured femur, and expected to have surgery to treat the fracture. It's important that they have at least one family caregiver who can be involved in their care during the hospital stay. However, those with certain serious conditions, such as active psychiatric issues or advanced dementia, are not eligible for the study. If you or a family member fit these criteria, you can expect to work closely with a trained caregiver who will help create a supportive environment to promote better recovery and reduce confusion during this important time.

Eligibility criteria
- Inclusion Criteria:
- • Males and females aged 65 years and older
- • Acute hospitalization in the orthopedic unit for fractured femur post-fall
- • Expected or actual surgical intervention for treatment of fracture
- • At least one family caregiver involved in supporting the patient during hospitalization
- Exclusion Criteria:
- • Documented conditions that have been clinically active during the previous 6 months: including psychiatric illness; cerebrovascular disease; brain malignancy; alcohol abuse; drug abuse
- • Terminal disease
- • Delirium
- • Inability to assess for delirium due to advanced dementia or sensory deprivation (marked impairment of vision or hearing) or any other condition that limits communication with the patient
