The DELICATE trial is studying a new method to reduce the pain of injections for patients undergoing carpal tunnel release surgery. Carpal tunnel syndrome (CTS) is a common condition that can cause pain and numbness in the hand due to pressure on a nerve. In this trial, researchers want to see if mixing a substance called sodium bicarbonate with a long-lasting local anesthetic can make the injection less painful. They will compare two groups of patients: one group will receive the regular anesthetic, while the other group will receive the buffered version. The main goal is to find out which method causes less pain during the injection and whether it affects overall pain control during and after the surgery.
To participate in this trial, you need to be between the ages of 65 and 74, have been diagnosed with carpal tunnel syndrome, and be scheduled for carpal tunnel release surgery. However, certain conditions may exclude you, such as previous injuries to the median nerve or some chronic health issues. If you join the study, you can expect to answer some questions about your pain experience and satisfaction after the surgery, as well as report on your pain levels and use of pain medications for a few days following the procedure. The researchers hope that this approach will lead to better pain management and improve your overall experience during surgery.

Eligibility criteria
- Inclusion Criteria:
- • Clinically and electrophysiologically diagnosed carpal tunnel syndrome
- • Patients who are scheduled for carpal tunnel release surgery.
- Exclusion Criteria:
- • Associated disease or conditions, including earlier injury to median nerve, cervical radiculopathy, cubital tunnel syndrome and other peripheral neuropathy
- • Chronic renal failure
- • Rheumatoid arthritis
- • Allergies to lidocaine
- • Pregnancy
- • Profound cognitive impairment
- • Previous ipsilateral carpal tunnel decompression
