This clinical trial is exploring a new way to care for elderly patients with certain medical conditions, like bladder infections, skin infections, and pneumonia, without needing to send them to the hospital. To be eligible for this study, participants must be 65 years or older, diagnosed with stable acute medical conditions, and living in their own home or a nursing home within specific areas in Denmark. They also need to have been seen by a doctor before joining the study and be able to understand and speak Danish. Participants can expect to receive either home care or standard hospital care, and researchers will compare the results to see which option is better for their health and well-being. This trial is currently recruiting participants, and it’s an exciting opportunity to help improve healthcare for elderly patients.

Eligibility criteria
- Inclusion Criteria:
- • The patient should be aged 65 and over
- • Diagnosed with a stable acute medical conditions e.g. cystitis, erysipelas, pneumonia
- • The patient should be living in their own home or a nursing home before entering the study
- • The patient should be residing in one of the three municipalities (Viborg, Skive and Silkeborg)
- • The patient must have been seen by the GP or the pre-hospital service in the case of disease in question
- • The patient should speak and understand Danish.
- • The patient should give informed consent to participate in the study.
- Exclusion Criteria:
- • The patient is unable to give written consent
- • The capacity of the municipal acut team care was fully utilised.
