This clinical trial is looking to improve care for children with complex medical needs by using a special telemedicine program. The study will see if using mobile devices at home can help parents and caregivers check important health signs like temperature and oxygen levels, listen to the heart and lungs, and look at areas like the skin and throat, all while staying connected with their healthcare team during clinic hours. The goal is to provide better support for these children, who often face multiple health challenges.
To be eligible for this trial, children must have had at least two hospital stays or one stay in the intensive care unit (ICU) in the past year. They also need to be considered at high risk for hospitalization without this specialized care. However, children with certain serious conditions, like major heart disease or active cancer, cannot participate. If your child is eligible and joins the study, you can expect to work with healthcare professionals who will guide you in using the telemedicine tools and help monitor your child's health from home. Eligibility criteria
- Inclusion Criteria:
- • Greater than or equal to 2 hospitalizations or greater than or equal to 1 ICU admission in the year before joining the comprehensive care program
- • Greater than 50 percent estimated risk of hospitalization without our care as judged by the medical director
- Exclusion Criteria:
- • Major heart disease
- • Mitochondrial disorders
- • Active cancer
- • Do-Not-Resuscitate (DNR) order
- • Patients receiving compassionate care
