This clinical trial, called the Transoral Endoscopic Thyroidectomy by Vestibular Approach (TOETVA), is studying a new way to perform thyroid surgery. The goal is to see if this technique can help patients with thyroid conditions, including thyroid cancer, while minimizing visible scarring on the neck. The trial is currently recruiting participants at the Instituto do Câncer do Estado de São Paulo and is open to individuals aged 18 and older.
To be eligible for the study, participants should have certain thyroid conditions, such as benign lumps or suspicious nodules that are less than 2 cm. They should also be motivated to avoid scarring on their necks and have thyroid growths that are not too large. However, individuals with previous surgeries in the head or neck area, certain thyroid issues, or specific types of cancer will not be eligible. Participants in this study can expect to undergo the new surgical approach, which aims to be less invasive and improve recovery times.

Eligibility criteria
- Inclusion Criteria:
- • history of hypertrophic scarring and/or motivation to avoid cervical scarring,
- • maximum diameter of the thyroid \< 10 cm and of the dominant nodule \< 6 cm,
- • estimated thyroid volume \< 45 ml
- • benign lesion (multinodular goiter, cyst),
- • indeterminate nodule (Bethesda III or IV),
- • suspicious nodule for well-differentiated thyroid carcinoma (Bethesda V and VI) \< 2 cm
- Exclusion Criteria:
- • history of previous surgery or irradiation in the head and neck region and superior mediastinum
- • evidence of clinical hyperthyroidism
- • preoperative recurrent nerve palsy
- • lymph node metastasis
- • extra-thyroid extension
- • plunging goiter
- • oral abscess
