This clinical trial is studying two methods for performing a procedure called percutaneous dilational tracheostomy in patients who are already on life support in the Intensive Care Unit (ICU). The main goal is to see if using a C-MAC videolaryngoscope, which provides a video view of the airway, is better than the traditional method of direct laryngoscopy for safely positioning the breathing tube. This is important because a misplaced tube can lead to serious problems, including difficulty in breathing and potential aspiration. The trial aims to ensure that healthcare providers can see the airway clearly and perform the procedure safely.
To be eligible for this study, participants must be intubated ICU patients who need a tracheostomy due to reasons like needing long-term mechanical ventilation or protection of their airway. However, patients younger than 18 or older than 85, those with certain neck or throat issues, infections, or bleeding disorders, and those who refuse to participate will not be included. If you or a family member qualify, you can expect to be monitored closely throughout the trial, and the study could help improve the way tracheostomy procedures are done in the future.

Eligibility criteria
- Inclusion Criteria:
- • ICU intubated patients that require a percutaneous dilational tracheostomy for clinical reasons (prolonged mechanical ventilation, airway protection or weaning failure).
- Exclusion Criteria:
- • Patients younger than 18 years and older than 85 years
- • Trachea and neck abnormalities.
- • Soft tissue infection in the neck.
- • History of neck surgery.
- • Coagulation disorders or changes in coagulation parameters (platelet count \< 50.000 mm3, an activated partial thromboplastin time 1.5-fold longer than the control value, and international normalized ratio \> 1.5).
- • Consent refusal for participating in the trial.
