This clinical trial is investigating how physiotherapy can help patients in the intensive care unit (ICU) who are at high risk of needing to be put back on a breathing machine after being taken off. The study compares a special program that includes both breathing exercises and early rehabilitation led by physiotherapists to the usual care provided in the ICU. The goal is to see if this combined approach can reduce the number of patients experiencing acute respiratory failure within a week after being extubated, which means having the breathing tube removed.
To participate in this trial, patients need to be between the ages of 65 and 74 and must be able to pass a test indicating they can breathe on their own without assistance. It's important that a family member or caregiver signs a consent form for participation. However, patients who have removed their own breathing tube, have a tracheostomy (a different kind of breathing support), are dealing with specific types of lung issues like acute lung edema, or have decided to limit or stop aggressive treatments will not be eligible for this study. Participants can expect to receive either the new physiotherapy approach or standard care during their time in the ICU, and their recovery will be closely monitored.

Eligibility criteria
- Inclusion Criteria:
- • Patients with spontaneous ventilation weaning test.
- • Consent form signed by the person support.
- Exclusion Criteria:
- • Patients with a self-extubation,
- • Patients with a tracheostomy,
- • Patients with acute respiratory failure for acute lung edema (OAP),
- • Patients with decision to limit or stop Active Therapeutics (LATA).
