The "Sport on Prescription" trial is studying the effectiveness of a program called "Cap Sport Santé 83" in helping patients with long-term illnesses get back into physical activity. This program is designed for people who have chronic conditions, like heart disease or obesity, and aims to encourage them to exercise safely and regularly. Physical activity can help improve health and prevent serious diseases, but many adults in France do not get enough exercise. By using this program, doctors can prescribe adapted physical activity (special forms of exercise tailored to individual needs) to help these patients.
To participate in this study, you need to be at least 18 years old and have a long-term illness or risk factors affecting your health. You should also be able to understand French and give your consent to join the study. Unfortunately, if you are pregnant or have certain health issues that make exercise unsafe, you won't be eligible. If you join the trial, you can expect support to help you safely increase your physical activity, which could lead to better health outcomes. This trial is currently recruiting participants, and your involvement could help improve the program for others in the future.

Eligibility criteria
- Inclusion Criteria:
- • Patient with a long-term illness of any type, or a chronic illness, or risk factors (such as hypertension or obesity), or persons with decreasing independance and a stabilised clinical condition
- • Aged 18 years old or over
- • Able to express his/her consent prior to participation in the study
- • Understanding french language
- • Affiliated to or beneficiary of a social security regimen
- Exclusion Criteria:
- • Patient with a contraindication to physical activity
- • Pregnant women
- • Patient under judicial protection (guardianship, curatorship...) or safeguard of justice
- • Any other reason that, in the opinion of the investigator, might interfere with the evaluation of the study objectives
