This clinical trial is studying a new tool called Personal Health Libraries (PerHL) to see how well it works for people who have recently been released from prison. The goal is to help these individuals manage their health information better, especially if they have chronic conditions like diabetes or high blood pressure. The trial is still in the planning stages and has not started recruiting participants yet.
To be eligible for this study, participants need to be at least 18 years old, speak English, and have at least one ongoing health issue. They also need to have been released from a correctional facility within the past year and currently engage in primary care, which means they see a doctor for regular health check-ups. Unfortunately, those with visual or cognitive impairments, language barriers, or plans to move away will not be able to participate. If you join the study, you can expect to learn more about managing your health and may help researchers understand how to improve health resources for people in similar situations.

- Inclusion Criteria:
- • 18 years or older
- • English-speaking
- • have at least one chronic condition
- • engaged in primary care in the community
- • have been released from a correctional facility in the past year.

- • planned relocation
