This clinical trial is studying how a special electronic tool can help doctors manage patients' blood sugar levels while they are in the hospital. The tool is designed to provide alerts and support for managing conditions related to diabetes, such as high or low blood sugar. Researchers want to see if using this tool can improve patient care and also find out how helpful doctors think it is for managing these conditions.
To participate in the trial, patients need to be adults aged 18 or older and currently hospitalized at specific Penn State Health facilities or outpatient clinics. Participants will receive the benefits of enhanced monitoring and support for their blood sugar management during their hospital stay. This study is currently looking for volunteers, and while children are not eligible, adults with diabetes or related conditions are encouraged to consider taking part.

- Inclusion Criteria:
- • Hospitalized adult (\>18 years) patients at Penn State Health, Hershey Medical Center, St. Joseph's Hospital, Hampden Medical Center, and Holy Spirit Medical Center
- • Ambulatory adult (\>18 years) patients at Penn State Health, Hershey Medical Center
- • Trigger of an alert or a disease management message
- Exclusion Criteria:
- • Children (\<18 years)
