This clinical trial is studying a new way to help childhood cancer survivors get the follow-up care they need after treatment. Many children who survive cancer face serious health issues later on, but less than 20% of them receive the recommended care from specialized clinics. The trial will test a combination of methods to improve care access and understanding, including online education, personalized information through health record portals, regular phone communication between cancer centers and primary care doctors, and in-person visits. The goal is to help these survivors stay healthy and ensure they receive the proper check-ups for any late effects of their cancer treatment.
To be eligible for this trial, participants must have been diagnosed with cancer before age 21 and have completed treatment (like chemotherapy or radiation) 2 to 4 years ago. They should be cancer-free and able to live for at least two more years. Participants can include children and teens, as well as their parents or guardians if they are under 18 and speak English or Spanish. The trial is currently looking for volunteers at specific hospitals across the country. If you join, you can expect to receive helpful education and support to manage your health as a survivor, making it easier to stay on track with your care.

Eligibility criteria
- Inclusion Criteria:
- • 1. Diagnosed with any cancer at age \<21 years
- • 2. Treated with chemotherapy and/or radiation
- • 3. 2.0-4.0 years status post-completion of all cancer-related therapy
- • 4. Cancer-free with a life expectancy of ≥2 years
- • 5. English- or Spanish-speaking (also applies to parent/guardian if patient age \<18 years)
- • 6. No previous attendance at a specialty survivorship clinic
- • 7. Followed at one of the 4 participating sites: Hackensack, University of North Carolina- Chapel Hill (UNC), University of Colorado Denver (CU), Miller's Children's and Women's Hospital Long Beach (MCWH)
- Exclusion Criteria:
- • Active medical problems severe enough to not be eligible for receiving survivorship care with primary care provider at the time of recruitment
