This clinical trial is studying how a patient’s breathing effort during mechanical ventilation affects their lungs and diaphragm. The researchers want to find out if allowing patients to take small breaths while on a ventilator can help protect their diaphragm and not harm their lung function. The study involves measuring how hard patients are breathing for up to eight days using a special balloon catheter, as well as performing daily ultrasounds of the diaphragm and lungs.
To participate in this trial, patients need to be over 28 days old, weigh more than 3 kg, and require mechanical ventilation for more than 48 hours. However, patients who have refused consent, had mechanical ventilation in the last two weeks, have certain diaphragm problems, or cannot have the balloon catheter placed are not eligible. If someone qualifies, they can expect daily assessments to monitor their breathing and muscle function during their time in the ICU. This research aims to improve how ventilation is managed in critical care settings.

Eligibility criteria
- Inclusion Criteria:
- • age \> 28 days
- • weight \> 3kg
- • requiring invasive mechanical ventilation
- • anticipated length of ventilation \> 48h
- Exclusion Criteria:
- • refusal of consent
- • mechanical ventilation ≤ 2 weeks before inclusion
- • known anatomical malformation of the diaphragm (including congenital diaphragmatic hernia)
- • contra-indication to the placement of a naso-gastric esophageal balloon catheter
