The clinical trial titled "My Interprofessional Care Team for Adherence and Research Engagement Disparities" (MI-CARE) is looking for ways to help patients better manage their blood pressure and take their medications as prescribed. This program focuses on patients who might be struggling with medication adherence, especially those who are African-American or Latino and have high blood pressure. The goal is to provide a coordinated care team that includes pharmacists and community health workers, making it easier for patients to receive the support they need to improve their health.

To be eligible for this trial, participants must be at least 18 years old, self-identify as African-American or Latino, and speak either English or Spanish. They should also be taking medication for high blood pressure, using at least five different medications, and not adhering to their hypertension medication 85% of the time or more. Participants can expect to work with a dedicated team that will help them manage their medications and overall health, contributing to a better understanding of how to address health disparities in these communities. Eligibility criteria

  • Inclusion Criteria:
  • 1. age ≥ 18 years;
  • 2. self-report cultural identity as African-American, or Latino;
  • 3. speak English, or Spanish;
  • 4. have medication-treated hypertension;
  • 5. use ≥5 chronic medications;
  • 6. have hypertension medication adherence \<85%; and
  • 7. able to provide informed consent.
  • Exclusion Criteria:
  • 1. if project staff conclude that a candidate participant is unable to comprehend the informed consent process (because he/she offers an inappropriate response to consent questions); or
  • 2. if the candidate is hostile or unwilling to follow project protocols.
