• • Type 2 Diabetes based on American Diabetes Association Criteria
• • Hemoglobin A1c ≥ 7.5%
• • Screen Positive on 2-item Food Security Screener

• • Unwilling to Attest to Release of Information to Mid-Ohio Food Collective
