This clinical trial is testing a new program called AI4CHRON, which uses artificial intelligence to help adults with chronic health conditions and mobility challenges improve their quality of life. The program focuses on making lifestyle changes that are personalized and family-oriented, aiming to support individuals who may struggle with managing their health due to disabilities or chronic diseases like heart disease, chronic lung disease, or Type 2 diabetes.
To join this study, participants need to be at least 18 years old, have one of the specified chronic conditions, and live with a permanent physical disability. They should also be able to read and speak English and have access to a smartphone or computer. Throughout the trial, participants can expect support from the program as they work on making healthier lifestyle choices, while also contributing to valuable research that could benefit others in similar situations. It’s important to note that people currently enrolled in other health programs, or those with certain severe health issues, may not be eligible to participate.

Eligibility criteria
- Inclusion Criteria:
- • 18 years or older
- • Diagnosis of heart disease, chronic lung disease, or Type 2 diabetes
- • Living with a permanent physical disability (i.e., spina bifida, multiple sclerosis, stroke,)
- • Have the ability to converse in and read English
- • Availability of a smartphone or computer
- Exclusion Criteria:
- • Current enrollment in any structured intervention
- • Severe cognitive impairment
- • Severe untreated depression in the past 6 months
- • Major cardiac event in the past 12 months
- • Uncontrolled blood pressure
- • Resting tachycardia
- • Renal failure
- • Severe peripheral neuropathy
- • Active treatment for cancer in the past 12 months
- • Having both visual and hearing impairments (either of them is fine)
