The ECMO-Free Trial is studying a way to safely and efficiently reduce the time patients spend on a life-support system called venovenous extracorporeal membrane oxygenation (VV-ECMO). This system helps patients whose lungs are not working well by providing oxygen to their blood. The trial aims to evaluate whether a daily check to see if patients are ready to stop using VV-ECMO can lead to better health outcomes and lower costs compared to the usual care approach, which varies widely between hospitals.
To participate in this trial, patients must be adults currently receiving VV-ECMO treatment at a participating hospital. However, those who are pregnant, under 18 years old, or have been on VV-ECMO for more than 24 hours are not eligible. Participants can expect to have their readiness for stopping VV-ECMO assessed daily, and the trial will gather important information to help improve care for future patients. This study is currently recruiting participants, and the findings could help shape how VV-ECMO is managed in hospitals across the country.

Eligibility criteria
- Inclusion Criteria:
- • Patient receiving VV-ECMO
- • Patient is located in a participating unit of an adult hospital
- Exclusion Criteria:
- • Patient is pregnant
- • Patient is a prisoner
- • Patient is \< 18 years old
- • Participant is receiving ECMO as bridge to transplant
- • Participant is receiving a hybrid configuration that includes an arterial cannula
- • Patient has received VV-ECMO for \> 48 hours
