This clinical trial, called "PREhabilitation of Candidates for Renal Transplantation," is studying a special program designed to help people who are waiting for a kidney transplant. The goal is to see if this personalized prehabilitation program can improve the health and strength of kidney transplant candidates, particularly those who may be considered frail. The study involves 128 participants, with half receiving the prehabilitation program—focused on physical fitness, nutrition, and mental well-being—over 12 weeks, followed by an additional 12 weeks of support. The other half will continue with standard care. Researchers will check how participants' overall health changes at different points during the study.
To be eligible for this trial, participants must be adults aged 18 or older who are on the kidney transplant waiting list at UMCG. However, if someone cannot read or speak Dutch, is waiting for a combined organ transplant, has a living donor transplant scheduled within three months, or is already in another lifestyle program, they cannot participate. This study aims to understand if this tailored approach can help improve health before a kidney transplant, ultimately leading to better outcomes for patients.

Eligibility criteria
- Inclusion Criteria:
- • 1. Adult kidney transplant candidates (≥18 years)
- • 2. Listed for kidney transplantation on the UMCG kidney transplant waiting list at the start of the study or wait listed during the inclusion period (October 2022 - March 2025).
- Exclusion Criteria:
- • 1. Inability to read and/or speak Dutch
- • 2. Combined organ transplantation (e.g., kidney+pancreas, kidney+liver)
- • 3. In case of living donor kidney transplant: a transplantation planned within 3 months
- • 4. Involved in a lifestyle intervention program
