To participate, individuals must be 65 years or older and have some concerns about their cognitive abilities, like memory or thinking skills. However, those who have already been diagnosed with dementia or live in permanent nursing facilities cannot join. If you decide to participate, you can expect to take the 5-Cog assessment and possibly receive enhanced care based on the results. The trial aims to ensure that when cognitive issues are identified, there are follow-up actions to improve patient care, particularly for those in communities that often face health disparities.

Eligibility criteria
- Inclusion Criteria:
- • 1. 65 years and older
- • 2. Presence of cognitive concerns
- • 3. English or Spanish speaking.
- • 4. Able to see and hear well enough to complete assessments.
- Exclusion Criteria:
- • 1. Prior diagnosis of dementia (documented in the electronic medical record or reported by physicians).
- • 2. Permanent nursing facility residents.
