ClinConnect Summary
This clinical trial is focused on helping adults who have both Type 2 diabetes and chronic kidney disease (CKD) prevent frailty, which is a condition that can cause weakness and make daily activities harder to perform. The researchers aim to develop and test a home-based program that encourages healthier eating and physical activity to improve the quality of life for these individuals. The goal is to help them maintain their independence and reduce the risk of falls or hospital visits.
To be eligible for this study, participants need to be between 50 and 85 years old and must have a confirmed diagnosis of Type 2 diabetes and CKD. Those who have certain conditions that affect their ability to exercise, have severe vision loss, or are currently on dialysis are not eligible to participate. If you join the study, you can expect to participate in activities designed to improve your diet and physical skills while receiving support from the research team. This is an exciting opportunity to take an active role in your health and well-being!
