This clinical trial is investigating a new way to help patients with obesity who are on breathing machines, known as mechanical ventilation, to breathe on their own more quickly. The researchers want to see if measuring something called transpulmonary pressure—essentially, the pressure inside the lungs compared to the pressure outside—can help doctors decide when it's safe to take patients off the ventilator. The goal is to reduce the time spent on the ventilator, which can lower the chances of developing complications like infections or muscle weakness.
To participate in this study, patients need to be between the ages of 65 and 74, be classified as obese (meaning they have a body mass index, or BMI, of 35 or higher), and have been on mechanical ventilation for less than 48 hours. However, certain health conditions, such as severe neurological injuries or specific gastrointestinal issues, may prevent someone from being eligible. If enrolled, participants will undergo additional monitoring and might receive a special device to measure their lung pressure, which will help guide their care.

Eligibility criteria
- Inclusion Criteria:
- • on invasive mechanical ventilation for respiratory failure for \< 48 hours
- • obese class II or higher (BMI greater than or equal to 35.0 kg/m2)
- Exclusion Criteria:
- • contraindications to esophageal balloon catheter placement including esophageal, nasofacial, gastrointestinal abnormalities and platelets \<10,000/microliter
- • known pleural disease such as persistent pneumothorax, pleural effusion, or pleurodesis
- • neuromuscular disease
- • requires chronic mechanical ventilation prior to enrollment
- • severe neurologic injury
- • known difficult airway
- • life expectance is less than 48 hours
