This clinical trial is studying a new approach to help adults struggling with opioid addiction who are also unemployed and living in poverty. The program, called the Therapeutic Workplace, offers participants a job where they can earn money while also encouraging them to stay drug-free. To support this, participants need to provide drug-free urine samples to earn higher wages. The goal of the study is to see if this approach can help individuals maintain their recovery from opioid use, find steady employment, and improve their financial situation.
To be eligible for the trial, participants must be at least 18 years old, currently receiving treatment with methadone or buprenorphine at a clinic called REACH Health Services, and must be unemployed. The trial will last for about a year, starting with a 4-week introduction period followed by 48 weeks of support from an employment specialist. Participants will be randomly assigned to different groups: some will receive usual care, while others will get extra financial incentives based on staying drug-free. This study aims to find out if these low-cost incentives can effectively support long-term recovery and employment for those facing opioid addiction.

- Exclusion Criteria:
- • Applicants will be excluded if they report recent hallucinations and if they are currently a prisoner.
