This clinical trial is focused on understanding and managing infections that can occur in patients with Acute Respiratory Distress Syndrome (ARDS) who are on Extracorporeal Membrane Oxygenation (ECMO), a life-support system that helps patients breathe and get oxygen into their blood. The researchers aim to find out how common these infections are, what types of bacteria cause them, and how resistant they are to treatment. They will also study how new antibiotics work in patients receiving ECMO to ensure that the medications are effective in this vulnerable group.
To participate in the trial, individuals must be over 18 years old, diagnosed with ARDS, and currently receiving ECMO. Participants will be monitored for infections during their treatment, and some will have their blood tested to see how well the new antibiotics are working. This study is important because it aims to improve the care of patients who are critically ill and at high risk for infections, helping doctors make better treatment decisions in the future. Eligibility criteria
- Inclusion Criteria:
- • Age \> 18 years
- • Diagnosis of ARDS (i.e., acute onset ( \< 7 days) from the known clinical insult of respiratory failure, with bilateral opacities - not explained by effusions, lung and lobar collapse, or nodules -, not fully explained by cardiac failure/fluid overload; with P/F ratio \<= 300 mmHg and continuous positive airway pressure \>= 5 cmH2O)
- • Ongoing ECMO
- Adjunctive Inclusion Criterium for the sole Secondary Objective:
- • - Antibiotic treatment with ceftazidime/avibactam, meropenem/vaborbactam, ceftolozane/tazobactam, or cefiderocol.
- Exclusion Criteria:
- • Pregnancy
- • Expected survival \< 24 hours.
