This clinical trial is looking at a new way to help people who have had a stroke feel better and become more active. Many stroke survivors experience something called post-stroke fatigue, which makes them feel tired and less motivated to move around. This study will test a program called "Physical Activity on Prescription" (FaR), where participants will receive personalized coaching through digital platforms twice a week for 12 weeks. The goal is to see if this coaching can help increase physical activity levels and reduce fatigue, ultimately improving the quality of life for stroke survivors.
To participate, individuals must have had a stroke between 1 to 5 years ago, live in the Gästrikland area, and have a certain level of post-stroke fatigue. However, those with severe strokes or significant depression may not be eligible. Participants can expect to engage in a supportive digital program focused on their individual needs, which may empower them to take charge of their health and well-being.

- Inclusion Criteria:
- • Stroke 1 to 5 years ago
- • Living in Gästrikland
- • Post Stroke Fatigue ( S-FAS ≥ 24)
- Exclusion Criteria:
- • Severe stroke (Modified Rankin Scale \>3)
- • Depression ( Geriatric Depression Scale \>5)
