ClinConnect Summary
This clinical trial is studying how a community-based patient navigator can help support pregnant women who are at higher risk due to social and health challenges. The trial will involve 540 pregnant women who are Black, English-speaking, and covered by Medicaid. These women will be enrolled before 20 weeks of pregnancy and will be randomly divided into two groups: one group will receive the navigation support, while the other will not. The goal is to see if this support can improve health outcomes for mothers and their babies and reduce health disparities.
To be eligible for the study, women need to be at least 18 years old, pregnant with a single baby, and have at least one unmet social need identified through a standard health assessment. Participants can expect to be part of a supportive program from mid-pregnancy until one year after their baby is born. This trial is currently recruiting participants, and if successful, it could lead to better care practices for high-risk pregnant women at Grady and other similar healthcare facilities.
