This clinical trial is looking at a special questionnaire called the Care Partner Hospital Assessment Tool (CHAT-AD), which is designed to help care partners—like family members or friends—of patients with Alzheimer's disease and other related dementias. The goal is to see if using this tool can improve the care that these patients receive after they leave the hospital. If you are a care partner for someone who has been hospitalized and needs assistance due to dementia, this study might be a good fit for you.
To be eligible for the trial, you need to be at least 18 years old and provide unpaid care to an adult relative or partner who has been hospitalized due to Alzheimer's disease or related dementias. The study will last for about 14 days, during which you will be asked to use the CHAT-AD tool. The trial is currently looking for participants, and it is open to anyone regardless of gender, as long as they speak English. Your involvement could help improve the support and care for people living with dementia after they leave the hospital.

- Inclusion Criteria:
- • Provide unpaid care to a hospitalized adult relative or partner to help them take care of themselves because of ADRD
- • 18 years or older
