This clinical trial is looking at how a special support program, called a Nurse Navigator Program, can help patients who are at high risk for being readmitted to the hospital after surgeries like hip or knee replacements. The goal is to see if providing education and guidance to these patients can help them stay healthy and avoid going back to the hospital. The study will also check how satisfied patients are with the educational support they receive.
If you are between the ages of 65 and 74 and are planning to have a hip or knee replacement, you may be eligible to participate, especially if you have certain health conditions like heart failure, chronic lung disease, or if you are on dialysis. However, if you have any issues that affect your ability to understand and agree to participate, you won’t be able to join. Participants in the study will receive additional education and support to help manage their health after surgery. This could be an important opportunity to improve recovery and overall satisfaction with the healthcare experience.

Eligibility criteria
- Inclusion Criteria:
- • - Require total or partial hip or knee replacement and have one or more of the following diagnosis: Heart Failure (HF); Chronic obstructive pulmonary disease (COPD); Dialysis; Alcohol Abuse; Low BMI.
- Exclusion Criteria:
- • - Decrease cognitive capacity to consent to the study.
