This clinical trial is studying a new approach to help older adults who have multiple chronic health conditions, like arthritis or diabetes, and also face difficulties in daily activities. The goal is to see if combining two types of therapy—behavioral activation (which helps people set and achieve health goals) and occupational therapy (which adapts activities to fit individual needs)—can improve the ability of these individuals to manage their health on their own. This is important because many older adults struggle to manage their health, which can lead to worsening conditions and increased hospital stays.
To participate in this trial, individuals must be at least 21 years old, have at least two chronic conditions, and experience some limitations in daily activities. They also need to be motivated to set at least one goal related to physical activity. Participants will be randomly assigned to either the new combined therapy or standard care, and the study will involve around 40 people living within 15 miles of the University of Oklahoma. This research aims to gather valuable information that could lead to larger studies and better support for older adults managing multiple health conditions.

Eligibility criteria
- Inclusion Criteria:
- • at least 2 chronic conditions
- • at least 1 functional limitation
- • intention to set at least 1 physical activity goal
- • live within 15 miles of University of Oklahoma (OU) Schusterman Center
- Exclusion Criteria:
- • life expectancy less than 6 months
- • progressive neurological condition
- • in active treatment for cancer
- • not English speaking
- • cognitive impairment
- • current or history of severe mental illness
- • in concurrent treatment with occupational and physical therapy
