This clinical trial is looking to find out which type of revisional weight loss surgery works best for people who have not had success with their initial weight loss procedure called sleeve gastrectomy. The three types of surgeries being compared are SADI-S, RYGB, and OAGB. The study will check how much weight participants can lose after these surgeries, how often complications happen, and whether their health conditions improve as they lose weight.
To be eligible for the trial, participants must have previously had sleeve gastrectomy and have gained some weight back, with a body mass index (BMI) of around 45 at the time of the new surgery. They can also have mild forms of acid reflux but not more severe cases. The trial is not yet recruiting participants, but it is open to individuals aged 18 and older. Those who join can expect to be closely monitored for their weight loss, any potential complications, and changes in their overall health after the surgery.

Eligibility criteria
- Inclusion Criteria:
- • Undergone primary laparoscopic sleeve gastrectomy in the past
- • Weight regain
- • defined as any increase in weight above the nadir as reported by the patient
- • BMI at the time of revisional surgery was around 45 kg/m2
- • weight regain was defined as an increase in BMI after bariatric surgery to exceed 35
- • With or without Gastroesophageal reflux disease (GERD) grade A and B o Patients with grade C or higher GERD, according to the Los Angeles (LA) classification \[7\] will be excluded from the study
- Exclusion Criteria:
- • Didn't follow preoperative consultation
- • Cannot give of sign informed consent
