This clinical trial is looking at how to improve medication management for older people who take many different medicines, which is known as polypharmacy. Specifically, it focuses on frail older adults (aged 75 and older) who use a special system that organizes their medications (called a multidose drug dispensing system). The study will compare a detailed medication review process, which includes strategies to safely reduce the number of medicines they take, with regular care to see which approach is more effective in helping patients lower their medication use and improve their health over six months.
To be eligible for this study, participants must be 75 years or older, use a multidose drug dispensing system, and take ten or more medications. Those who have had a similar medication review in the past year or who live in nursing homes won’t be able to participate. If chosen, participants can expect to meet with a trained pharmacist for a thorough review of their medications, including discussions with their doctor and follow-up support. This trial aims to help older adults manage their medications more effectively and reduce any unnecessary medicines, ultimately enhancing their quality of life.

- Inclusion Criteria:
- • using a multidose drug dispensing system
- • hyperpolypharmacy patient (using 10 drugs or more)
- • age: 75 years or older
- Exclusion Criteria:
- • received a CMR in the previous 12 months
- • incapacitated
- • patients for whom the general practitioner is not the primary practionar
- • expected lifespan of less than six motnhs
- • patients who live in a nursing home
