This clinical trial is studying the effectiveness of a new smartphone application designed to help patients with chronic respiratory or cardiovascular diseases improve their health. The program lasts for 12 weeks and aims to enhance exercise capacity, reduce shortness of breath, increase muscle strength, and improve overall quality of life for participants. It’s especially important for people in South Korea, where it can be challenging to access traditional rehabilitation programs.

To be eligible for this trial, participants should be adults aged 20 to 80 who experience some level of shortness of breath and have been diagnosed with a chronic respiratory condition (like severe asthma or chronic obstructive pulmonary disease) or a chronic heart issue (such as heart failure or a history of heart attacks). Participants will use the smartphone app to guide them through their rehabilitation exercises and monitor their progress. It's important to note that individuals who have had a recent severe worsening of their condition or who cannot follow the rehabilitation program won't be able to join the trial. Overall, this study aims to provide a convenient way for patients to get the support they need to improve their health from the comfort of their homes.

Eligibility criteria

  • Inclusion Criteria:
  • Dyspnea symptom \>= mMRC 1 or NYHA II
  • adults between 20 years and 80 or years
  • Chronic respiratory disease
  • FEV1/FVC \< 0.7 or FEV1\< 0.8 of predicted value in pulmonary function test
  • bronchiectasis in more than one lobe on chest computed tomography
  • FVC or DLCO \< 0.8 of predicted value in pulmonary function test
  • Chronic cardiovascular disease
  • Reperfusion Therapy for angina pectoris or myocardial infarction
  • Heart failure with reduced ejection fraction (LVEF \< 50%)
  • Exclusion Criteria:
  • history of acute exacerbation within 4 weeks
  • unable to comply rehabilitation program
  • not suitable for study on decision by duty physician
  • no consent
