This clinical trial, called SCALED, is studying how to improve the care of adult patients with traumatic brain injury (TBI) by using a special system designed to help doctors make better decisions about preventing blood clots, known as venous thromboembolism (VTE). The goal is to adapt an existing tool to better support patients with TBI in seven different hospitals. By doing this, researchers hope to ensure that patients receive consistent and effective care, reducing the risk of complications from blood clots.
To participate in this trial, you need to be at least 18 years old and have been admitted to the hospital with a TBI. However, if you die within 24 hours of being admitted or if your care plan is focused solely on comfort during the first few days, you won't be included in the study. If you join, you can expect to receive care that follows the latest guidelines for preventing blood clots, while helping researchers understand how well this system works in practice.

- Inclusion Criteria:
- • age 18 and older
- • admitted with a TBI
- Exclusion Criteria:
- • Patients who die within 24 hours of hospital admission and patients documented as "comfort cares" during the first 72 hours of hospitalization or with mild TBI will be excluded from final analysis.
