This clinical trial, called Patient Initiated Note About Goals (PING), is exploring how a program that encourages patients to share their personal health goals can improve care for people with chronic diseases. The goal is to see if giving patients the opportunity to express their needs and priorities during their annual wellness visits can lead to better health outcomes.

To participate in this trial, you need to be between 76 and 70 years old and have been referred for an annual wellness visit at a participating clinic. Unfortunately, if you do not speak English, you will not be eligible to join. If you choose to take part, you can expect to work with healthcare providers who will help you set and communicate your health goals, making sure your care aligns with what matters most to you. This trial is currently recruiting participants, and your involvement could help improve healthcare for others facing similar chronic conditions.
