Patients with stroke with uncontrolled glucose levels have higher mortality and poor post stroke outcomes. Controlling diabetes and other associated risk factors are effective ways to prevent initial strokes as well as stroke recurrence.   • Inclusion Criteria:
  • Age 60 years and more
  • Diagnosed with hypertension, diabetes mellitus and cardiac diseases.
  • Able to communicate with health team, free from mental problems, and agreed to participate in the study were included.
  • Exclusion Criteria:
  • unable to communicate with health team.
