This clinical trial is looking at how a special support program can help people who have had a stroke or a transient ischemic attack (TIA) when they leave the hospital. The main goal is to see if this person-centred care approach makes it easier for patients to manage their health after discharge, understand their medications, and feel more involved in their care. Participants will be compared to those receiving regular care to find out if the new support program leads to better outcomes, such as fewer repeat strokes, improved understanding of health information, and less burden on caregivers.

To be eligible for this trial, participants need to be adults aged 65 and older who have experienced a stroke or TIA and are getting ready to go home from the hospital. They should be able to give consent to join the study. Throughout the trial, participants will receive help from healthcare professionals who will guide them in managing their recovery and preventing future strokes. This study is currently recruiting participants, and it's a great opportunity for those looking for additional support during their transition from hospital to home.

Eligibility criteria

  • Inclusion Criteria:
  • patients who have had a first time ever or recurrent stroke or TIA; who are to be discharged from the participating hospitals to home and referred to a neurorehabilitation team for continued rehabilitation; and who are able to give informed consent by themselves.
  • Exclusion Criteria:
  • unable to give informed consent, due to e.g., severe aphasia or dementia.
