This clinical trial is studying a new technique called MGPOCUS-assisted bronchoscopy to help doctors perform a procedure known as endotracheal intubation (ETI) more effectively, especially in patients who may have a difficult airway. ETI is important because it helps patients breathe when they are unable to do so on their own, but it can be challenging in some cases. The study will compare the traditional method of using a bronchoscope with this new MGPOCUS approach to see which one leads to faster and more successful intubation, while also monitoring for any complications and how satisfied patients are with the process.
To participate in this trial, individuals need to be between 18 and 85 years old and require intubation due to potential airway difficulties. They must provide informed consent and be willing to participate. However, certain people will not be eligible, such as those with specific neck issues, a history of certain surgeries, or conditions that could lead to complications during the procedure. If someone decides to join, they can expect to be randomly assigned to one of two groups, and their experience and outcomes will be carefully monitored throughout the study. This trial aims to improve the safety and effectiveness of a critical medical procedure for patients in need.

Eligibility criteria
- Inclusion Criteria:
- • Aged between 18 and 85 years old.
- • Requiring ETI.
- • Anticipated DTI meets one or more positive findings in the airway evaluation, including history, examination, and appropriate investigations of anatomy .
- • Signed written informed consent.
- • Willingness for the primary anesthesia team to participate.
- Exclusion Criteria:
- • Anterior neck lesions (masses, lacerations, or subcutaneous emphysema).
- • A history of neck operation or tracheotomy.
- • Allergies to ultrasound coupling gel.
- • At risk of pulmonary or cardiovascular complications during intubation with flexible bronchoscope, including severe hypoxemia, severe pulmonary hypertension, unstable or severe obstructive airway disease.
- • At risk of bleeding during bronchoscope, including anticoagulants or coagulopathy, renal insufficiency, and superior vena cava syndrome.
- • High risk of aspiration.
- • Current pregnancy.
- • Patient unable to cooperate (for awake intubation).
